If you purchase SFM's products and services, we collect billing and credit card information. This information is used to complete the purchase transaction.

Children Under Thirteen
SFM collects personally identifiable information from children under the age of thirteen. SFM collects this information for the following reason(s): SFM counsels children over the age of 8. There is an adolescent intake form that guardians are required to submit before counseling children. The adolescent form has HIPAA-compliant information similar to the adult intake form.
We provide information about our personal data practices for children on our home page and wherever we knowingly collect personal data from children on our website. If you are under the age of thirteen, you must ask your parent or guardian for permission to use this website. From time to time, SFM may contact you via email for the purpose of providing announcements, promotional offers, alerts, confirmations, surveys, and/or other general communication.
