Complete the Detailed Intake (for ages 16 & up) OR complete the Youth Intake (for ages 15 & under). You can complete the form one of two ways:

Option 1: Complete the form digitally on your computer and email it to tammy@flcwy.org

Option 2: Print the form, complete it by hand, and return it at your first appointment
